COVID-19 Testing Consent (Secondary and 6th Form)

We are working to keep our academies as safe as possible. The Trust will continue to ask students to continue to test twice weekly at home. Along with the other protective measures we are taking, these tests will support the continuation of face-to-face education.

We would like all students to take an on-site lateral flow device (LFD) test in the academy at the start of term in January 2022. More information will be available on each academy’s website regarding the arrangements for this.

Before students can take an on-site test, we must have a record of consent to take part in the testing process. If you are happy for your child to be tested, please fill in the form below. Please complete one form for each child. You may have given consent in the past, but we must ensure our consent records are up-to-date, so do please complete the form. Your child cannot be tested if we have not received the consent form.

This consent form is for participation in tests at an asymptomatic testing site designed to detect asymptomatic coronavirus cases. Anyone experiencing symptoms should follow government guidelines to self-isolate, even if they have had a recent negative lateral flow test.

Consent relates to the following groups of students and staff as follows:

  • For students younger than 16 years – this form must be completed by the parent or legal guardian. Please complete one consent form for each child you wish to participate in testing.
  • Students over 16 who are able to provide informed consent – can complete this form themselves, having discussed participation with their parent/guardian if under 18.
  • For any student over 16 who does not have the capacity to provide informed consent – this form must be completed by the parent or legal guardian.
